Medical Exam at St. Lukes (Philippines)

The beneficiary can get the medical exam one or two weeks before the visa interview at the US Embassy. This will give St. Lukes time to submit the medical report to the Embassy before the interview schedule.


Medical Fee:

Php 17,025.00 (for 15 years and older)

Php 9,583.00 (for below 15 years)



Requirements:

  1. Valid passport

  2. Copy of the biographic page of the passport

  3. Copy of the St Lukes Online Registration Confirmation

  4. Copy of the NVC Letter with the case number

  5. Copy of the Visa Interview Appointment Confirmation

  6. Copy of the DS160 Confirmation Page (for Fiance(e) Visa)

  7. Copy of the DS260 Confirmation Page (for immigrant visa)

  8. Four (4) identical 2"x2" colored photo with white background

  9. Minor applicants must be accompanied by a parent or legal guardian


Tips on Medical:

  1. The medical exam usually takes about two (2) days.

  2. Arrive at St. Lukes early in the morning.

  3. If you have had some medical issues, bring medical reports.

  4. If you lost your vaccine records, it's fine. St. Lukes will provide age-appropriate vaccinations

  5. All required vaccinations are included in the medical fee.
